Track your patient’s status with “Pick up at center”

Issue

<p>Use the <strong>Pick up at center</strong> feature in Accu-Chek Care to enable patients to pick up test strips at the center and help you monitor their testing routine.</p>

Solution

<h5>How to set up Pick up at center feature?</h5><ol><li><strong>Create a guideline: </strong>Set up the patient’s testing routine in the <strong>Guideline</strong> tab. The system will then calculate when the patient should collect test strips at the center. <a href="https://support.accu-chek.com/care/professionals/strip-management/how-manage-my-patients-testing-routine-using-guidelines">Learn how to create a guideline.</a></li><li><strong>Set up alerts:</strong> Configure alerts for hypos, hypers, or other warnings to identify patients who may need additional support.</li><li><strong>Keep patient data updated: </strong>Ensure the patient uploads data from their meter before any delivery. This keeps their status accurate and up to date.</li><li><strong>Check the status color: </strong>After the data upload, the Delivery status color will update, indicating whether the guideline is being followed or if an alert has been triggered.</li><li><strong>Schedule a visit (if necessary): </strong>Depending on the status, the software may require the patient to visit their healthcare professional before receiving any further test strips.</li></ol>

  • When can patients pick up new test strips?

    <p>Patients can collect new test strips based on their <strong>Delivery status</strong> color:</p><ul><li><strong>Green: </strong>They have used all or most of the prescribed test strips and can continue their testing routine.</li><li><strong>Orange: </strong>They have consumed a considerable amount of the prescribed strips or have exceeded one or more thresholds, triggering an alert.</li><li><strong>Red (cannot collect strips): </strong>They have used fewer strips than prescribed but still have enough stock, have used more strips than prescribed and may need a visit to review their routine or they have gestational diabetes and are outside the range of their due date.</li><li><strong>Gray (cannot collect strips): </strong>There is no data to calculate status for the current period. This can occur if an upload is pending, not yet due, or a delivery has just been made.<br>&nbsp;</li></ul>
